Treatment of the postphlebitic syndrome by means of valve transplantation

Abstract

The aim of this study was the determination of indications for the transplantation of the competent valve of the brachial-axillar vein to the distal part of the femoral vein in post-thrombotic syndrome patients. Since 1990, we have performed 23 such operations (16 males and 7 females). The patients average age was 43.5 (ranging between 20-74). Descending flebography had shown a 3rd and 4th grade venous reflux, according to Kistner's classification. Treatment results were estimated by means of clinical normalization and venous circulation hemodynamics. The patients clinical state was evaluated by means of the Clinical Score. Normalization of venous circulation hemodynamics was evaluated by means of photoplethysmography (Venous Refilling Time) and Duplex scanning methods (Venous Reflux Index). During the physical examination, we observed statistically positive changes. The photoplethysmographic test revealed normalization of hemodynamic parameters. We demonstrated that in chosen cases of the post-thrombotic syndrome, transplantation of the brachial vein segment with a competent valve to the distal part of the femoral vein might be necessary.
