Histological lesions and liver biochemistry depending on HCV genotype and viral load in patients (pts)with chronic hepatitis C:preliminary results

Background:To assess the association of hepatitis C virus (HCV) genotypes and viral load with some biochemical parameters and liver histological lesions.Material/Methods:Thirty four pts with chronic hepatitis C (CHC)(15/44.1%male,19/55.9%female,mean age 48.2 years) were investigated.CHC diagnosis was confirmed by the presence of anti-HCV (EIA 2-nd generation,ABBOTT)and HCV RNA (AMPLICOR HCV,ROCHE). HCV viral load and HCV genotypes were detected by PCR amplification (AMPLICOR HCV MONITOR Test version 2.0,ROCHE; INNO-LIPA, INNOGENETICS). Biochemical liver tests were performed in all pts,while liver biopsy –in all,but one with hemophilia,and scored according Ishak and METAVIR.According to HCV genotype all pts were divided into two groups:gr.1 –28/82.4% pts had HCV genotype 1, gr.2 –6/17.6%–HCV genotype 2 and 3 (1/2.9%and 5/14.7%,respectively).The biochemical parameters (activity of ALT,AST, ALP,aGT and bilirubin)and extent of histological lesions (HAI,fibrosis,lymphoid aggregates and follicles,steatosis,bile duct damage)were compared in pts within and between groups depending on viral load –800000IU/ml. Results:Within gr.1 slightly higher,but not statistically significant scores of HAI and grade of fibrosis (6.0 ±1.8 vs 7.4 ±1.6;3.0 ±1.3 vs 4.0 ±1.0)were found in pts with higher viral load,while higher prevalence of steatosis in those pts was statistically significant (15.8%vs 66.7%,p800000 IU/ml)viral load.Higher scores of HAI and fibrosis as well as prevalence of lymphoid aggregates and follicles in pts with genotype 1 and higher viral load did not reach statistical significance.There were no differences of mean values of biochemical parameters depending on viral load neither within nor between 2 pts groups.
