Case report of a 33-year old male admitted to the hospital due to retrosternal pain lasting for several hours. The ECG on admission demonstrated a profound T-wave inversion in the precordial leads. During hospitalization cardiac enzymes remained within normal limits. We excluded a myocardial infarction. A selective coronary arteriography demonstrated an arterial fistula originating from the LAD and communicating with the left ventricle.
